The student will complete this assignment to ensure that eligibility requirements for the course have continued to be met.
The student will complete the Disclosure Statement Form Template to acknowledge and verify charges, convictions, citations, etc. and to grant permission to the field education staff of the BSSW program to release information from the Disclosure Statement Form Template for the purpose of arranging field placements.
Prior to beginning the process of securing a field placement, the student will complete a Self-Assessment. The student will explore his/her fit for the profession, compliance with professional values and ethics, and general readiness to enter the field placement. There are no right or wrong answers to the questions being asked but the student should carefully and honestly consider responses to each question and fully reflect on what that means for him/her while moving forward in the program.
The student will consider the following when completing this assignment:

The student will fill out the chart in the Field Education Self-Assessment Template.
The student will utilize provided information to complete a resume revision in preparation for an internship or employment.
The student will begin to plan for next semester in preparation for the commitment of field placement.
Competency Self-Reflection Assignment
This assignment provides students the opportunity to reflect on their current skills and understanding of each of the nine social work competencies. Students use a provided template to evaluate their perspective of their competence for each and then identify strengths and areas for growth. Students create SMART goals for the competencies in which they’d like to grow for their junior field experience.
The student will not submit anything for this assignment. The student’s assigned field advisor will indicate whether the proposal has been approved, after the entire proposal (Forms #1-6) have been submitted correctly and completely.
The student will not submit anything for this assignment. The student must successfully complete all assignments in this course and receive full approval on a field placement proposal to be eligible for Gate Approval.
The student will complete placement proposal forms and demonstrate that the placement will meet agency, task, supervision, and schedule requirements according to CSWE and LU guidelines. Each quiz will contain 1 short-answer question and have no time limit.
The student will complete quizzes throughout the course that cover information from each assigned Learn section. Each quiz will contain 1-15 multiple-choice, true/false, essay, and/or short-answer questions and have no time limit.
