Course Description

This course emphasizes the coordinated approach to the delivery of substance abuse and addiction services. Effective case management focuses on the whole person, addresses other client psychosocial problems, and is associated with treatment retention which leads to better outcomes. The role of effective documentation, referral, and relapse prevention are highlighted.

Rationale

Becoming a professional in the field of substance abuse and addiction services will require a thorough knowledge of effective case management and relapse prevention. Developing an effective case management plan and relapse prevention plan are key components of successful addiction counseling. This course is designed to provide the student with foundational knowledge in order to be competent in case management assessment and relapse prevention planning specific to the addiction population. Further, this course contains information that the student will need to know in order to pass most state addiction certification and licensure exams.

Proficient knowledge of aspects of addictive behavior, coupled with sound biblical values, will permit a more active and excellent informed dialogue and critique of present case management and relapse prevention practices.

Discussions are collaborative learning experiences. Therefore, the student is required to provide a thread in response to the provided prompt for each forum. Each thread must be at least 400 words, incorporate at least 1 citation in current APA format, and demonstrate course-related knowledge. Community Resources and Emergency Contacts Assignment

Using the template for community resources or a similar chart, research and list resources in the student’s own city. Use the emergency contacts form to make an emergency phone contact list for the student’s own city. The assignment must include at least 20 resources and contacts, and must include a 30–50-word paragraph for each entry. Title this assignment "Community Resources and Emergency Contacts.” CLO 1, 2, 3

Case Management Plan Assignment

Using the Case Study provided, the student must write a 4–5-page Case Management Plan in current APA format that determines the key issues presented by the client and a potential approach to managing this client’s presenting symptoms. The Case Management Plan must include at least 3 scholarly sources in addition to referencing the course textbooks and the Bible. In addition, the student must include some of the community resources that he/she researched for the Community Resources and Emergency Contacts assignment. CLO 1, 2, 4, 5

Relapse Prevention Plan Assignment

Using the Case Study provided, the student must write a 4–5-page Relapse Prevention Plan in current APA format that determines the key issues presented by the client and a potential approach to managing this client’s presenting symptoms.
