Report An Incident

If you have experienced or witnessed any of the following types of prohibited conduct, please let us know by submitting a report.

  • Sexual Harassment
  • Quid Pro Quo
  • Hostile Environment
  • Sexual Assault
  • Dating Violence
  • Domestic Violence
  • Stalking
  • Sexual Exploitation
  • Retaliation and Interference with Process
  • Complicity
  • Attempt to Commit Prohibited Conduct
  • Discrimination (based on an individual’s legally protected class status)
  • Discriminatory Harassment (based on an individual’s legally protected class status)

Can I Make an Anonymous Report?

The university accepts anonymous reports of Prohibited Conduct and encourages individuals to provide detailed information, as our investigators’ ability to investigate or respond may be limited without enough information for a thorough investigation.

Responsible Employees 

All responsible employees who receive information about Sexual Misconduct are REQUIRED to report that information to the Title IX Coordinator by using the Report an Incident form.

Amnesty 

You will NOT be disciplined for Liberty Way violations that took place in circumstances where you experienced, witnessed, or committed sexual misconduct. Learn more about amnesty.
