Assignment Accommodation Request
Assignment Accommodation Request Form
- Use this form to request approval for an assignment accommodation request – especially for physical education courses.
- Please submit a doctor’s note if there is a physical reason you cannot complete an assignment due to an injury or other physical limitations.
- You must log in as the parent/affiliate POC to submit this form, not as the student.
Course Extension Request
- Starting in term 202338, the Course Extension Fee Schedule is being updated. If a student needs more time to complete a course beyond the allotted time of 41 weeks (full year) or 22 weeks (semester), a 30-day extension can be requested by completing a Course Extension Form.
- If approved, a fee of $50 per course will be charged for the first extension, $100 for the second, and $150 for the third (if extenuating circumstances exist). A new request form must be completed for each extension beyond the first
- Extensions will be approved based on the student’s progress in the course at the time of the request at the discretion of LUOA Academics.
- Quarter, Enhanced, and Dual Enrollment courses do not qualify for extensions.

- Use this form to request approval to be exempt from a course or grade at LUOA and move on to the next course or grade.

Credit Appeal Request
- Use this form to request approval to transfer credits received for a course in a previous institution into LUOA.
- You must log in as the parent/affiliate POC to submit this form, not as the student.

- Transcripts for traditional LUOA K-12 courses are free for our families.

Dual Enrollment Transcript Request Form
- Use this form to request electronic or mailed Dual Enrollment transcripts.
- Transcripts that show only the college credit hours that have been received for Dual Enrollment courses cost $10 for the initial transcript and $1 for each additional transcript requested in the same order.
- You must log in as the student to submit this form.
Student Complaint Form
General Policy for Online Academy Student Complaint
Complaints not specifically addressed by policies in any portion of the above student support areas should be directed to the Liberty Online Academy Student Advocate Office by following the complaint procedure below and completing the Student Complaint Form. The Student Advocate Office will assess the complaint, identify the appropriate person(s) to assist the student, and facilitate contact with the student and appropriate office(s) to ensure resolution of the complaint. It is important to note that this policy is ONLY for complaints not addressed by other stated policies. Additionally, the Student Advocate Office may be contacted in cases where a student is uncertain on how to proceed with a complaint, or if a complaint remains unresolved after a reasonable amount of time.
- Students may submit a formal complaint about an issue occurring during their current or previous term of enrollment.
- Only complaints submitted via the online complaint form will be considered official complaints. Complaints submitted via other forums will not be reviewed.
- You must log in as the student to submit this form.
- Student Advocate reviews the complaint and accompanying documentation.
- Student Advocate, if necessary, assigns the complaint to another university office (Student Accounts, Academics, etc.) for research/decision-making.
- Complaint decision is rendered based on research findings.
- The decision-making party will rule on the complaint and update the online complaint form.
- The student is notified via email of the decision.
- If the student is not satisfied with the outcome of their initial complaint, they may appeal within 5 business days of the rendered decision by providing additional support/documentation for review.
- A second review of the complaint will be completed by a higher authority whose decision will be final.

As your time with Liberty University Online Academy comes to an end, there is some important information you need to know. Whether you plan to attend the Commencement ceremony or not, there are steps that you must take in order to graduate with a high school diploma.
