INSTRUCTIONS

Student Verification Request Form


1. A Student Verification Request Form should be completed when verification of enrollment needs to be obtained.

2. Please complete each section of the Student Verification Request Form to ensure prompt processing.

3. Verify that the return address or fax number is current and written legibly on the Student Verification Request Form.

4. Completed Student Verification Request Forms can be e-mailed to sneely@liberty.edu from your Liberty University account. If a Student Verification Request Form is submitted from any other e-mail account, the form must contain an electronic signature. All other Student Verification Request Forms can be presented to the Registrar’s Office.
