This form is required in conjunction with the CSER Application form if the CSER supervisor is not currectly approved  by the Center for CSER.  To determine the approval status of a particular supervisor, please contact the Center for CSER directly.

It is required that all CSER Supervisors agree to the terms listed on this form and sign it. The form must then be returned to the Center for CSER and approved before one can serve as a Supervisor of Liberty students.  This form only needs to be filled out once by a Supervisor, and should be returned by the student to the Center for CSER.
