This form is to be used for Special Projects (CSER 399-001), Church Extension (CSER 279-001), or Personal Ministry (CSER 282-001) CSERs. For clarification, this form is to be filled out by all students wishing to complete their CSER through a non-profit organization or non-profit agency not listed as a pre-approved CSER (See below listings for these CSERs) which would then be considered Special Projects. This form is also to be used by all students wishing to complete their CSER with a local church (considered a Church Extension) other than Thomas Road Baptist Church (TRBC). Finally, certain ministry opportunities for TRBC not specifically listed in the 200 Series listing below would be considered a Personal Ministry and would also require the use of this form.
This form must be completed by the student, supervisor and then approved by the Center for CSER before the student can be registered and receive credit toward their graduation requirement.
NOTE: The CSER Supervisor Contract form may also be required in conjunction with this CSER Application form. If you are behind on your CSER requirements, you may make up your CSER requirements in later semesters by completing more than one CSER in a semester. However, you may only complete a maximum of two (2) CSERS with one CSER registration. IT IS MANDATORY THAT STUDENTS APPLY FOR APPROVAL THROUGH THE CENTER FOR CSER BEFORE CREDIT WILL BE GIVEN FOR ANY MORE THAN ONE CSER PER SEMESTER. You may apply for approval by writing “Double” on the top of this form.
