The following links provide the basic forms that are central to the CSER Registration and Evaluation process.  These documents are provided for your convenience.  Alternatively, you may pick up these forms up in the Center for CSER  located in Green Hall, Room 1880.  ALL COMPLETED FORMS MUST BE SIGNED BY THE CSER SUPERVISOR AND TURNED INTO THE CENTER FOR CSER FOR APPROVAL AND FURTHER PROCESSING.
